Family memberships are for up to five family members. I understand that as a member, I am entitled to vote at the annual meeting if I am 18 years of age or older . If an annual meeting is not held in a physical location and voting is conducted by ballot, either by regular mail or online at the ACE of Norwood website, I certify that the above address is my accurate mailing and/or email address and ballots may be received by me at that address. I understand if I do not provide a correct mailing address, my submission will constitute a waiver of notice of any meeting or ballot in which I may be entitled to participate.
